That was the whole point of the takeover in the 70’s! The respect, power and wealth that doctors used to get was usurped by insurance companies. Now THEY make all the decisions; even medical ones!
My sister works in Healthcare and it’s sad how often patients have to change a treatment or medication that is working for them because their insurance decides to just switch things up on them. Or like how a doctor can be covered, but not their facility, and vice versa. Just whack.

So here’s a quick breakdown:
The government(Medicare) comes up with a list of prices that procedures should cost based on lots of factors such as where the hospital or doctors office is located, how urgent the care is, what others doctors are charging for the same service in that area and expertise, ect.. They enter it into a website and “suggest” the doctor use it as a guideline to charge patients.
Now, if the provider signs a contract with the insurance company or become in network, the provider agrees not to charge the patient or the insurance company more than what is the agreed upon amount.
If the provider decides not to sign up with insurance providers, not join the network, aka out of network, the provider can charge any amount they like. The doctor sends the bill to the insurance company for reimbursement but the insurance company will only pay what the patient agreed to under their out of network clause when they signed up for the plan based on their policy and the patient is responsible for the rest.
